North Country Assistance Programs and Services

New York State Programs and Services
New York State, in partnership with Google.org, developed a tool to help New Yorkers in need connect with appropriate services with a three-step questionnaire. Learn more about this tool on the Updates page.
New York State’s Office of Temporary and Disability Assistance (OTDA) hosts a website for individuals and families in need to determine if they are eligible for benefits, to track their claims, and update their accounts.
CoC Provider Resources
CoC Service Providers can visit the Points North Housing Coalition’s Resource Library to access documents, resources, and other materials to assist agencies in serving clients within the Continuum of Care.
According to the National Alliance to End Homelessness, adults and children in families make up about 33 percent of the homeless population. The PNHC connects homeless families with services and assistance programs in New York’s North Country spanning across Jefferson, Lewis, and St. Lawrence counties. 
This nation’s heroes sacrificed to keep us safe, but now they need us to keep them safe. Through access to housing, healthcare, and income assistance programs, the PNHC assists homeless veterans in getting back on their feet. 
Regardless of who they are, the PNHC is committed to connecting those in need with the available homelessness services in the community. To accomplish this, the Coordinated Entry system helps providers in Jefferson, Lewis, and St. Lawrence counties dispense their services to most vulnerable population through a prioritization list. This list is generated from the homelessness assessments completing by service providers in the PNHC. 
Homeless individuals are not alone in the North Country because the PNHC provides access to a network of providers in Jefferson, Lewis, and St. Lawrence counties that connect the individuals with the assistance programs and services that they need. 
Homeless and runaway youth have a door to turn to in New York’s North Country. The PNHC connects homeless youth with assistance services and programs that help stabilize the youth in the moment of crisis.
