Coordinated Entry Assessment Request
Please fill out this form so we may contact you to complete a Coordinated Entry Assessment. This assessment identifies your needs based on a series of questions. Completing the assessment may place you on a priority ordered waiting list to receive services from the community. However, you are not guaranteed to receive services just by completing an assessment.
Coordinated Entry staff will reach out to conduct the assessment over the phone or via a virtual call. If you are unavailable at that time, staff will schedule an appointment to conduct the assessment. All information related to the assessment is kept completely confidential.
