This resource library is primarily for CoC providers participating in the Coordinated Entry System. Introduction to Coordinated Entry
View the Introduction to Coordinated Entry Guide to learn about the basics of the Coordinated Entry process. This resource was developed by the Points North Housing Coalition Coordinated Entry staff to help onboard new providers and refresh existing providers with the basics of Coordinated Entry.
Housing First
Learning about the Core Principles of Housing First. This resource was developed as an excerpt from a webinar hosted by HUD and USICH on June 26, 2014. The organizers conducted a presentations that provides useful information about two important evidence-based interventions: Housing First and Rapid Re-Housing.
Coordinated Entry Users
Authorized User Form
There are two ways for a Points North Housing Coalition CoC Provider to request access for a user to the Coordinated Entry list. Agency users may chose to use the electronic form to send directly to the Coordinated Entry Staff or they may download the .pdf version of the form and submit the form according to the directions on the document.
After the Authorized User Form is received and processed by staff, users will receive access to the Coordinated Entry Priority List. Please access the Coordinated Entry Priority List using the instructions provided by the Coordinated Entry Manager.
CE Committee
The Coordinated Entry committee is comprised of members in the community supporting the success of the Coordinated Entry program. The CE Advisory Committee is responsible for updating policy and procedures, ensuring compliance, and supporting the overall structure of the full Coordinated Entry committee for Continuum of Care.
Memorandum of Understanding
Coordinated Entry users must receive the Policy and Procedures Manual for the Points North Housing Coalition Coordinated Entry System. It is the CE user’s responsibility to read and comply with the policies contained in the Manual and any revisions made to it.
Assessments
Reference Guide
Staff completing Homeless Assessments can use the assessment tool reference guide to find sample language for interviewing, privacy notices, and other information related to the Coordinated Entry process.
Assessment Tools
CoC providers use the Coordinated Entry Housing/Homeless Assessment Tool to identify a client’s needs and to begin the Coordinated Entry process. This resource helps Coordinated Entry staff score each client’s vulnerability based on the information collected in the tool. This tool is available in both printable PDF format or fillable PDF format.
The last three pages of the assessment tool includes the Client Release which addresses how information about clients may be used and disclosed at Providers as well as informing the client of their rights over their information.
This tool is also available without the Client Release as seen to the right.

ESG CV Homelessness Prevention Assessments
ESG CV CARES Act funding is available in each County that makes up the Points North Housing Coalition CoC. The prevention programs that access these funds are designed to prevent an individual or family from moving into an emergency shelter or living in a public or private place not meant for human habitation. This is addressed by providing relocation and/or stabilization services and/or short/medium term rental assistance. Operators of the ESG CV projects must use the Coordinated Entry Prevention Listing to work with the highest scoring, most vulnerable households that meet program qualifications.
